New Patient Registration

 

Welcome to Central Uxbridge Surgery, if you are a new patient who is looking to register with us, you can do so by completing our online registration form below. 

We need to check that you live within our surgery's boundary area. To check this simply type your postcode in the checker below. If you are within our boundary you will be given a link to our registration form.

Our patient registration policy does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition.

We are able to provide you with only urgent or emergency medical services until you have joined the practice.

How To Register?

Complete a New Patient Registration Form

It's simple to register with the practice, simply fill out our secure online form and attach copies of your proof of address and identity.

Register as a New Patient

Alternatively, you can register by visiting the surgery. Please bring along your medical card to the surgery if you have one.

Or, if not, we have a simple form for you to fill in available from our support staff.

  • You will be asked to provide proof of address (tenancy agreement, council tax bill, utility bill or bank statement)
  • Your NHS number
  • passport or photo ID

These documents are not a mandatory requirement and you will not be refused registration if you cannot produce them.

New Patient Health Check

We offer and encourage all patients to have three yearly health checks with the practice nurse and the support staff will advise you of the arrangements for making an appointment with one of the practice nurses.

When seeing the doctor or nurse for the first time it would be helpful to bring a list of all medication and serious illnesses including operations you may have had.

The support staff will ask you to complete a new patient information sheet which will assist with this information required.

Once the registration form and checkup are done we can register you with NHS England to obtain your medical notes from your previous GP and offer you the full range of NHS Health Services.

Registering for Online Services

You can use the Internet to book and cancel an appointment with a GP, request repeat prescriptions and view aspects of your medical record online.

If you wish to register for this service, please use the application for online access form.

Temporary Services

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Your Named GP

All Patients registered with the surgery will have been allocated a named accountable GP who will be responsible for your overall care. However, you can book appointments with any of the GPs at the surgery. If you have a preference to which GP you are allocated, the practice will make reasonable efforts to accommodate your requests. If you wish to know which GP has been allocated to you, please ask reception at your next visit.
